The Healthy Start Enhanced program aims to eliminate disparities in birth outcomes in San Bernadino County, CA through the provision of direct support to families during pregnancy and postpartum and engaging partners in the broad mission of achieving birth equity with a special focus on Black communities and the social determinants of health.
The Consortium Manager will establish a robust Community Consortia (formerly known as Community Action Networks or "CANs") comprised of diverse multi-sector partners that advise and inform HSE activities, as well as develop and implement action plans to improve perinatal outcomes within San Bernardino County.
The Consortium Manager facilitates the development of culturally responsive referral partnerships to address diverse participant needs, collaborating with healthcare providers, mental health services, WIC, SNAP, TANF, and organizations focused on housing and food insecurity. The consortia must also comprise critical stakeholders such as Title V, public health departments, hospitals, health centers, substance abuse agencies, community leaders, and service agencies.
